Birthday Application – 2019-2020
Make a public announcement of your child’s or favorite teacher’s birthday! Tell everyone about it for only $30.00. The BCS birthday package includes personal yard announcement, morning shout out and a free dress certificate.
Complete the form above and:
1. Drop CASH envelope “Birthday Announcement” in PTO Mailbox at the front desk. -or-
2. Purchase on PTO Store and email form to [email protected].
Please keep in mind:
- Application must be received at least 7 days prior to announcement.
- Applications will be accepted on a first-come, first-served basis.
- Payment must be included with application. Cash or Credit Only
- Scheduling conflicts may occur on some dates.
We will try to accommodate all requests. Payment(s) will be returned if the requests cannot be granted.
